Investigation Summary

At approximately 7:00 a.m. on November 21, 2005, Employee #1, a member of the grounds maintenance staff, was assigned to mow the front nine holes of a golf course. At approximately 8:30 a.m. he was observed riding a club car 2000 golf cart, Model Number RG0217, Serial Number 142200. He was driving northerly on an 8-ft-wide cart path. The path is approximately 31 ft from the edge of the canal. There is a steep drop off, approximately 10 ft to the water and then a gradual angled towards the middle. The cart was found approximately 8 ft from the edge of the canal at approximately 10:30 a.m., unattended. A head count was made and Employee #1 was not found. They entered the water but could not find him. EMS was called and at approximately 11:00 a.m., Employee #1 was found approximately 10 to 15 ft in front of the cart in the canal. Employee #1 died.
