Employee Is Killed When Run over by a Forklift

On October 22, 2005, Employee #1 was standing on the forks of a forklift that was being driven by a coworker. The coworker who was driving ran over a storm drain causing Employee #1 to lose his balance and fall. The coworker could not stop in time and he ran over Employee #1 with the forklift. Employee #1 suffered fractures and died.
