Investigation Summary

Investigation Nr: 201203106
Event: 12/22/2005
Employee's Fingers Are Amputated by Chain and Sprocket

At approximately 10:15 p.m. on December 22, 2005, Employee #1 was performing preventative maintenance on a running laundry ironer, when he was caught in a chain and sprocket drive. Employee #1 was greasing the various bearings, shafts and chains and had just finished in one area when he decided to remove access grease. Employee #1 used a towel and tried to make it smaller, when he wrapped the access material around his hand and reached into the running equipment. The moving chain caught the towel pulling it and Employee #1's hand through the chain and sprocket system. The tip of Employee #1's thumb and middle fingers were amputated, and the index and middle fingers were crushed. Employee #1 was hospitalized and treated for his injuries.
