Investigation Summary
On September 22, 2005, Employees #1, #2 and #3 were engaged in a bridge repainting operation and were working from cable supported Catenary scaffolding. The connecting clamp device failed at field weld and scaffold collapsed. The employees were not having their fall protection connected to the horizontal life. All three employees fell 18 feet or greater to the lower ground level. Employee #1 suffered multiple fractures and was hospitalized for ten days. Employees #2 suffered fracture of neck. Employee #3 had multiple fractures. Both Employee #2 and #3 were treated at the hospital and released the same say.
