Investigation Summary
On October 31, 2005, a truck driver was hooking a trailer onto a tractor. Initially the trailer was too high, so he lowered the trailer and then attempted a second time, resulting in the fifth wheel of the trailer to become jammed. As the truck driver used a round pipe to un-jam the retainer, Employee #1 offered assistance. Both Employee #1 and the truck driver, after multiple attempts, were unsuccessful. The truck driver indicated to Employee #1 that he was going to drive up to the loading area and get a larger bar to help free the retainer. Employee #1 then stated that he needed to get on the road to begin his delivery run. The truck driver got into his truck and observed Employee #1 get into his truck and close the driver's-side door. The truck driver then covered the round pipe in towels and laid it on the floor of his cab. He checked his mirrors, released his air brakes, put the truck in gear, and rolled forward. Observing that he had run over something, the truck driver exited his truck and saw Employee #1 lying on the ground near where his rear tires had been. Employee #1 was killed due to severe crushing injuries to his chest and head. The accident investigation revealed a pry bar was lying near Employee #1 that was not with him when he was assisting the truck driver.
