Investigation Summary
On April 27, 2006, Employee #1 and a coworker were cleaning a concrete pumping machine, a TK-40. This machine has a tank, with a slow mixing blade, and the top is covered with grating. A concrete truck dumps concrete into this tank. At the bottom of this tank are two tube openings. A tube diverter covers one of these openings at all times and switches back and forth. The hydraulic pump pulls concrete out of the tank into a tube and forces it to its desired destination. After each use, this machine must be cleaned out to avoid damage to the machine. Employee #1 was leaning head first down into the tank of the pump with his right arm in the pump diversion tube cleaning the concrete out. The coworker was standing on the flipped-out grate, 3 to 4 ft off of the ground, of the pump spraying water into the tank assisting Employee #1. The remote control was sitting on the top of the grate next to the feet of the coworker. The coworker went to get off of the grate and contacted the remote control activating the tube diverter, the machine was energized and in remote mode. This sheared off Employee #1's hand, and he was hospitalized.
