Investigation Summary
On January 6, 2006, Employee #1 was working on a packaging line that fills 5-pound paper bags with milled rice. After filling, the bags enter a "stitcher" that closes the top of the bags. As the bag is stitched closed, it moves between two chain-like compression guides and exits the "stitcher." The packaging conveyor was shut down for the line clean-up including the removal of excess paper material associated with the sewing unit. When Employee #1 could not reach material using needle nose pliers, she reached around the side of the "stitcher" where the bags exit the unit and inserted several fingers of her left hand into a slot of a clear guard in an attempt to reach the excess material. Employee #1 was not certain what actually grabbed her finger, but she felt something and pulled it out. Employee #1 suffered an amputation to the tip of her index finger, and was treated but not hospitalized.
