Employee Is Killed When Run over by Forklift

At approximately 2:30 p.m. on October 19, 2006, a coworker was operating a forklift bringing stacks of pallets in from outside and setting them inside dock Number 3. A coworker was driving a Cat Model RC 60, Serial Number 31A01840, heavy duty forklift. The conditions outside were approximately 40 degrees, overcast and the ground was dry. The forklift appeared to have very little, if any maintenance, done to it. The coworker had to back out of the dock because semis were located in docks Number 2 and 4 and he could not turn around. As he was backing up around the semi located in dock Number 2, he struck Employee #1 who was walking around the semi from the other direction. The coworker ran over Employee #1's chest before realizing what had happened. Employee #1 was rushed to the hospital and died.
