Employee Is Killed When Run over by Dump Truck

At approximately 10:25 a.m. on April 7, 2006, Employee #1, a truck driver, was driving a dump truck, a 1997 Mack. He was delivering stones to a construction site and was repairing a switch that activates the tailgate of the dump body. While standing on the driver's side between the dual axels, the tail gate opened and the truck rolled forward over Employee #1 when the stones were released. Employee #1, was killed.
