Event: 07/30/2007
Employee Dies from Blunt Force Trauma to Head

Employee #1 started work at approximately 7:00 a.m. on July 30, 2007. Employee #1's work station was at the rear of the plant and he was working alone. There were no witnesses as to what happened. At approximately 7:30 a.m., the owner of company was making morning rounds to greet employees. The owner discovered Employee #1 lying unconscious on the floor. As stated above, there were no witnesses as to what happened, but Employee #1 was working with a step ladder and a large metal fabricated structure. The ladder was still standing, but the structure was lying on its face. The structure could have been laid down by Employee #1, or could have unexpectedly fallen. Employee #1 was transported to a hospital where he died of the head injuries.
