Investigation Summary

Investigation Nr: 202345476
Event: 08/25/2005
Employee Dies from Fall

On August 25, 2005, Employee #1, working as part of an asbestos removal crew, was working from an 11-ft-high rolling scaffold with a coworker. Employee #1 climbed up off of the scaffold and onto the topside of an overhead ceiling so that he could get to another area where more material needed to be removed. While he was up in the ceiling, he stepped on a 2-ft by 4-ft fluorescent light fixture, and the fluorescent light fixture came down from the ceiling, causing Employee #1 to fall approximately 13 feet. He hit the back of his head on the concrete floor. The victim was transported to the hospital, where he died.
