Employee's Finger Is Amputated

On April 3, 2005, Employee #1 and his coworkers were running parts on a Vibromatic press in manual mode. The TSS Vibromatic machine had been programmed to over ride interlock gates and switch to two man operation. Employee #1 was placing a new part in the press when his coworker tripped the press to run amputating Employee #1's right index finger. No guard was on press to prevent access to the point of operation.
