Employee Is Killed When Pulled Into a Conveyor System

On November 14, 2005, Employee #1 was working on a conveyor system. He had performed this same task for the past seven days. The particular system that the employee was working on had not been tagged/locked out. The conveyor system began running and pulled Employee #1 into an opening, killing him instantly. The coworker in charge of locking and/or tagging out equipment was a different person than the one performing the same function in the previous days.
