Investigation Summary
On December 10, 2008, Employee #1, a machine operator, was cleaning the rollers on a flanging machine (Boldrini, Serial Number 23066) by holding a rag on one roller at time while they rotated. The rag was caught between the moving rollers and pulled in his left hand. He was taken to a local clinic, where the tip of a left finger was amputated. The employer manufactures a large variety of flanged metal parts used for numerous purposes, mostly as tank tops and/or bottoms. On the day of the accident, Employee #1's supervisor was in the workplace but not present at the accident site.
