Employee Falls, Suffers Concussion

At approximately 12:00 p.m. on December 3, 2008, Employee # 1 was instructed by his immediate supervisor to clean out the grill section of the car wash to remove debris build up. Employee # 1 spent approximately 20 minutes cleaning out the grill section of the car wash and proceeded to leave the area. Employee #1 slipped on the concrete floor and fell backwards hitting his head on the ground. Employee # 1 was taken to a nearby hospital by ambulance. Employee #1 suffered a closed head injury with concussion and a small hematoma on the posterior aspect on his scalp. Employee #1 was not hospitalized.
