Employee Dies from Heart Attack While Operating Welder

On December 15, 2008, Employee #1 was working in a welding shop using a Lincoln squirt welder, Model Number LN-8, Serial Number U1990408751, when he collapsed on the floor and died from a heart attack. Further investigation revealed that he had a prior history of multiple heart attacks.
