Investigation Summary
At approximately 1:30 p.m. on July 25, 2008, Employee #1, an experienced facilities and maintenance worker, began trimming an overgrown tree in the customer service area of the dealership, per the direction of his supervisor. Employee #1 asked his supervisor for a coworker to assist him in this task. Employee #1 used the flat roof of a single story building, 10 feet above the ground, as a work platform, and began cutting the tree. He began working by himself, not waiting for the coworker to arrive. With one large limb left to cut from the tree, Employee #1 rigged the limb with rope in such a way that he thought the limb, once cut, would be suspended from the tree over the roof area. Still without help, Employee #1 stood on the edge of the roof and made the last cut in the limb. It swung toward him, pushing him off of the roof. Employee #1 fell onto a small dirt patch adjacent to the building, striking his head and face. He suffered a skull fracture, spinal injuries and a fractured eye socket from the fall. He was hospitalized for treatment.
