Investigation Summary

At approximately 2:30 p.m. on August 24, 2008, an employee of a rehabilitation center for mentally impaired facilities was instructed by her direct supervisor to control Group Number 2 of clients and help them to get dinner and go to bed. At approximately 7:00 p.m., the employee took Group Number 2 to dinner and proceeded to take them to bed at approximately 8:00 p.m. One of the clients wanted to use the restroom. As the employee escorted the client to the restroom, the client began running back to his room. The employee began to follow the client down the hallway as a chair leg caught the employee's right foot, and she fell onto the concrete floor using her hands and knees to brace her. The employee then called out to her direct supervisor, and they both went to the onsite health department. The employee drove herself to a nearby hospital. She suffered a cut to her upper lip and swelling to the left side of her face. The employee was hospitalized.
