Investigation Summary
On August 5, 2008, Employees #1, a journeymen carpenter, and a coworker were assigned to build screen walls and install them to conceal a cellular antenna on the rooftop at a two story commercial building site. Employee #1 was climbing a 24-ft Werner Extension Ladder, Extra Heavy Duty Commercial Use, Model Number D6124-2. The fiberglass extension ladder was placed on a tile deck, on the second floor landing, leaning against the roof line. The ladder was extended approximately 20 ft to the roof. Employee #1 was approximately half way up, when the ladder slid backwards. Employee #1 fell on the tile decking, fractured his left leg, and sustained a head injury with internal bleeding. Employee #1 was transported to UCI Medical Center, Orange, California, where he was hospitalized.
