Investigation Summary
On July 25, 2008, Employee #1, a runner, was moving goods from docked trailers into designated areas of the freezers and removing orders for loading in a large cold storage warehouse for mostly frozen foods. He was using a Crown rider pallet jack, Model Number PE3540-80, Serial Number 6A190310, to accomplish that task. That was the pallet jack, he customarily used. One set of pallets did not have the exact rack location specified, as was usually the case, and Employee #1 began to visually scan the upper racks in the warehouse for space. His pallet jack veered slightly to the right, while he was looking for space, and he accidentally struck one of the rack uprights. His leg was pinned and crushed between the upright and the steering arm of the pallet jack. Employee #1 was able to move the pallet jack sufficiently to unpin his leg and call for help. He was hospitalized.
