Employee Lacerates Finger While Using Table Saw

On May 23, 2008, Employee #1 was cutting wood on a table saw (Model Number 72A and Serial Number 0104072053) without using a guard or a push stick. The wood was 4 in. by 4 in. by 21 in. in dimension. He walked behind the table saw to pull the piece of wood through the saw, when the wood kicked back and his hand pushed forward. His hand was struck by the blade and pulled into the saw. At the hospital, he received medical treatment for severe finger lacerations and included surgical procedures, which involved the amputation of his left ring finger from the second knuckle to the fingertip. Following the medical care, he was hospitalized.
