Investigation Summary

On February 21, 2008, Employee #1 was part of a crew plastering a new two-story custom home. He had just completed the morning scaffold inspection. Employee #1's written statement affirmed that the inspection had found the scaffolding to be in safe order. He then climbed to the scaffold's second-level planks (12 ft above the ground). While preparing to scratch coat a balcony, he fell to the ground. During the fall, Employee #1's face may have struck a fence rail. He was hospitalized for a jaw fractures. Employee #1 was alone at the time of the accident and did not recall the fall or what caused it.
