Investigation Summary
On January 25, 2008, Employee #1 was directed by Employee #2 to remove a tarp that was covering a stack of lumber. Employee #1 was then raised by the forks of the forklift, operated by Employee #2, the operator of a forklift onto a load of wood where he was to remove the tarp while he was elevated 7 feet from the ground. Employee#2 left to pick up a load and then drove back to the load where Employee #1 had completed removing the tarp. Employee #2 directed Employee #1 to step onto the load he was carrying in order to lower him to ground level. The placement of the forks underneath the load was unbalanced on the forks. Employee #1 stepped down onto the right of the carried load causing the load to tip. The two loads on top of the base load slid over and crushed Employee #1 against another load to the Operator's right. Employee #1 was hospitalized for approximately 2 weeks. Employee #1 underwent several surgeries to repair fractured bones to his left knee, left lower leg, his right ankle and vertebrae.
