Employee Falls from Ladder, Later Dies

On October 3, 2007, Employee #1 and a coworker were replacing a heating element on a small, 4-gallon, wall-mounted water heater. This heater was approximately 9.5 ft above the floor, and had been out of order for several days. There was no hot water and the cold water had been mostly drained out of this tank. The employees had also disconnected the power by pulling out the plug from the wall receptacle. Employee #1 was using an 8-ft, non-conductive fiberglass step ladder to access this equipment while the coworker was holding the ladder. As Employee #1 was removing the old heating element from the tank, some water squirted out on his face, which startled him and caused him to fall off the ladder. Employee #1 fell approximately 6 ft onto a concrete floor. The ladder did not fall with him. Employee #1 suffered head trauma and died in the hospital 6 days after the accident.
