Investigation Summary
At approximately 2:00 p.m. on December 5, 2006, Employee #1 was standing on a plywood platform making adjustments to the overhead louvers of a Alumatherm thermal heat treatment oven (Model Number 12, Serial Number DWG NO 3705-12) when the platform gave way causing Employee #1 to fall 9 ft 6 in., into the empty quenching sump floor. Employee #1 suffered a retroperitoneal hematoma and multiple rib fractures on the left side. Employee #1 was hospitalized.
