Employee Falls from a Ladder Later Dies

On December 4, 2006, Employee #1 was connecting cable TV service to a home. He had to access a cable tap next to a wooden pole approximately 25 ft high. There was a metal rope strand running along with a TV signal cable, which was tied between the top of this pole and another pole, approximately 30 ft away. Due to the slopping grade level, this strand had approximately 10 degree slant towards the downhill direction. He setup an extension ladder with its top end hooks looped around this metal strand next to the uphill pole. While he was on the ladder, the ladder slid over the strand next to the uphill pole. The top end of the ladder then slid over the strand towards the downhill direction, and he was thrown off to the asphalt surface, approximately 25 ft. He suffered a severe head injury and died two days later at the hospital.
