Employee Injures Foot When Caught Under Pallet

On April 5, 2006, Employee #1 and two coworkers were unloading a trailer docked at the delivery dock. Employee #1 and a coworker were stacking boxes on a pallet, which had been placed on forklift forks within the trailer. The other coworker, a forklift operator, moved the forklift a little forward toward the employees. When he drove the forklift forward, Employee #1's left foot was caught under the pallet, and three of his toes were crushed. Employee #1 suffered a crushing injury to his left foot and fractured three toes. He was transported to a medical center. At the hospital, he underwent surgical procedures to repair the damage and stabilize the toe fractures. After the surgical procedures, he was hospitalized for four days and was then released.
