Employee Is Killed in Tire Explosion

At approximately 10:30 a.m. on February 27, 2008, Employee #1 and coworker were repairing wheels for a Kenworth T800 tractor and eight wheel frameless dump trailer rig. The employee had patched and inflated a tubeless 11R22.5" Dayton tire and the coworker observed him walk the tire/wheel assembly through the service area to mount it on the tractor which was located outside. The coworker saw him with the single piece trailer rim on the floor using a leverage bar to mount the tire over the bead. Approximately five minutes later, a loud 'boom' noise was heard with an impact sufficient enough to shake the internal wall of the adjacent retail show room. The loud noise was created by an 18 in. radial rupture along the side wall of the tire and the employee was found several feet away from a front service door. The wheel assembly was observed wobbling as it came to a rest between the employee and the door. Emergency personnel responded to the scene at approximately 10:40 a.m.. Employee #1 was killed by blunt trauma to the thorax. There was a restraining device rack and a spliced 6 ft by .5 in. air hose (to clear employees of the wheel trajectory) available but unknown if either were or were not facilitated. The air line assembly for tire inflation lacked a clip on chuck and in line pressure gauge/regulator assembly. Toxicology results for the employee indicated the presence of Cannabinoids but it was ultimately unknown if the results to such a post mortem examination directly related/contributed to the accident event.
