Investigation Summary
On April 26, 2007, Employees #1 and #2 were delivering roofing materials to a private residence. Upon arrival at the delivery site, both employees exited the cab of the truck. They determined that they needed to use the truck's conveyor to remove the materials from the truck; they intended to place the materials on top of the residences roof. Employee #1 manned the truck in order to operate the truck's conveyor, while Employee #1 remained on the ground. Employee #1 raised the boom, with the roofing materials connected, inadvertently contacting the 7,200 volt power lines above. Employee #2, who had his hand resting on the truck, became trapped by the electricity while. Employee #1 jumped off the truck and tried to free Employee #2 from the truck. After three attempts to free Employee #2, Employee #1 was finally successful. Both Employees #1 and #2 were taken to a nearby hospital, where they were hospitalized and treated for their injuries. Employee #2 underwent surgery to have a portion of his toes removed. The accident investigation reported that the electric lines were at least 18 feet above the ground, and determined that Employees #1 and #2 did not appropriately evaluate their situation. They should have either repositioned their truck or not used the conveyor to deliver the shingles to the roof. The employer had to shut the power off in order to remove the truck from the electrical lines.
