Investigation Summary
On October 6, 2005, Employee #1, a tractor-trailer driver, was delivering a load of 12-inch by 40-ft pipes, which were bundled into three packs, each pack weighing 2,780 pounds, to a retail municipal plumbing supply business. The trailer was one-third unloaded, and Employee #1, who was undoing the tie downs for the cargo, was observing the forklift operator using a Hyster model N110XM diesel powered forklift to unload the truck and stack the pipe in piles. The pile being built with the unloaded pipe bundles was 9- to 12-feet high, and stacked without interlocking, cribbing, pyramiding or other means to prevent collapse of the pile. After unloading three bundles, the forklift operator was maneuvering at the front of the tractor trailer with another bundle. As Employee #1 walked from the rear of the trailer towards the front of the tractor, the front rows of the pile collapsed, crushing and killing Employee #1.
