Employee Punctures Lung in Fall

Employee #1, a foreman for a water tower painting company, was on the top riser of a ladder on inside a water tower. He had placed the rounded rails of the upper section of an extension ladder on the steel ring at the top of the riser which was approximately 6-inches wide. The ladder slipped and Employee #1 fell approximately 36 feet onto the concrete base between the dry tube and the outer wall of the riser, which was a space of approximately 42 inches. Employee #1 was hospitalized for treatment of multiple fractures of the upper arm, fractured femur, fractured pelvis and punctured lung. Employee #1 was not wearing fall protection.
