Investigation Summary
On August 31, 2005, Employee #1 was working for an HVAC firm. He was on a flat roof, removing the old gutter, when he fell head-first about 8.5 feet onto a balcony banister. From there, he fell an additional 9 feet to the rocky ground, and then he slid another 8.5 feet to the edge of the woods. He sustained head injuries in the fall, and he was killed.
