Investigation Summary
A forklift operator was accidentally pulled into the wire drawer machine, when his left hand reached into the machine to remove some burnt lubricant. His glove was caught on the wire that was being stretched, and it pulled his hand and arm into the machine. He struck his head against the machine before he landed on the ground. The spindle was traveling about 850 fpm.
