Employee Is Killed While Unloading Trailer

On October 18, 2005, Employee #1 was unloading a Link-belt LS-98A, 40-ton crane from the back of a beam trailer. Workers placed an oak flotation mat (weighing between 3,000 and 5,000 lbs) on each side of the trailer. The crane was then lowered onto the mats. Employee #1 was standing on the right side of the trailer and signaled for the crane operator to begin walking the crane off the back of the trailer. When the crane was about 2 ft from the edge of the trailer, the cable on the flotation mat became entangled in the crane's tracks which caused the mat to be lifted as the crane continued to move. When Employee #1 observed the mat lifting, he signaled the crane operator to stop, but was unsuccessful. While Employee #1 was running away, the cable snapped. The mat then swung, struck him, and landed on his head and torso. Employee #1 was transported to a local hospital, where he died from blunt force injuries.
