Investigation Summary
On October 6, 2005, Employee # 1 and Employee # 2 were working from a scissor lift on a concrete slab floor that had pot holes throughout the area. Employee # 1 was the operator, and had the scissor lift elevated approximately 24 feet, while moving the scissor lift around the concrete slab floor. Employee # 1 allowed the left front wheel of the scissor lift to slip off into one of the pot holes, causing the scissor lift to turn over, slamming the carriage platform the two employees were working from into a column post and onto the concrete floor. Employee # 2 landed on top of Employee #1. Employee # 1 sustained severe head injures and later died at the local hospital. Employee # 2 received minor injures and was released from the hospital on the same day.
