Investigation Summary
On July 13, 2005, Employee #1 was standing on top of an 8-ft-tall block wall. He was pouring concrete inside the block cells using a hose connected to a concrete pump. While attempting to remove excess concrete poured in the cells, he tripped and fell onto a finished concrete floor. At the time, he declined medical attention, but that night, he became ill. He was taken to a hospital, where he was pronounced dead on the following day. He had sustained multiple skull fractures.
