At approximately 1:15 p.m. on October 8, 2005, Employee #1, fell approximately 15" feet from a metal truss to a concrete pad. Employee #1 was pronounced dead at 5:17 P.M. at UK Hospital as a result of a cerebral herniation received in the fall.
