Employee falls from truck and later dies

On September 15, 2005, Employee #1 dumped a load of rock from a quarry at a ready-mix company. Upon completing the dump, Employee #1 drove his truck to the rear of the building. Employee #1 was observed standing on the entry platform on the driver's side of the truck dusting the cab of his truck with a short-handled mop. Employee #1 was found a short time later severely injured on the ground beside the truck. Employee #1 fell 38 inches to the ground and later died as a result of his injuries.
