Employee Fractures Back in Fall from Beam

On July 13, 2005, Employee #1 was working as an ironworker. He was on a beam, trying to level it, when his sleever bar slipped, causing him to fall. He fell approximately 15 ft. and sustained a fracture to his head. He was hospitalized. There was no fall arrest system in place at the time.
