Investigation Summary
Employee #1 was parking an aerial lift between two buildings with limiting operating room. The swing control switch on the lift was located near the key ignition and it is suspected that the employee intended to turn off the lift after parking it and accidentally activated the swing control switch. He was found with his head pinned between the lift and a wall. He was killed.
