Investigation Summary
On October 17, 2005, Employee #1, a legally blind, mentally-disabled paper shredding machine operator, was reaching into the cutter blades at the rear of the machine to free a jam. Another operator, who was unaware of Employee #1's location, started the shredder. Employee #1's hand then became caught in the cutter blades. He was hospitalized for an amputation.
