Investigation Summary
At approximately 10:32 a.m. on July 27, 2005, Employee #1,of Jones Truck & Spring Repair, Inc., was repairing a truck for a customer. The next day the customer came to the facility to pick up his truck. Before leaving, he noticed a boot on the drive shaft had not been placed correctly and wanted it corrected. One of the counter employees instructed the customer to drive his truck to the back bay area, and he informed the foreman in the back that the customer was coming and what needed to be done. Employees saw the customer, and Employee #1 was talking and walking in the bay area. Employee #1 was run over by a truck. The foreman that had been called was the first to see that the employee had been ran over by the truck. There were no witnesses to the accident. It is thought that the Employee #1 told the customer he would place the boot on (no more than a two minute job). Employee #1 went under the truck to place the boot. At the same time, the customer got back into his truck and began to move it, backing over the employee's head and chest, causing blunt crushing injuries.
