Investigation Summary
On October 4,2 005, Employee #1 was using a forklift truck to remove a scissor lift from a flatbed trailer. The forklift was necessary, because the rear wheels on the scissor lift were locked. Employee #1 had the forklift truck on the bed of the trailer to move the scissor lift. He placed himself between the truck and the lift in order to slide the forks of the forklift truck toward the center. The flatbed trailer was not level, though, and the scissor lift "slid" down the trailer and crushed Employee #1 between the forklift truck and the scissor lift. He sustained several injuries, and he was killed.
