Investigation Summary

Investigation Nr: 201692928
Event: 05/07/2008
Employee's Eye Socket Is Fractured When Struck by Forklift

On May 7, 2008, Employee #1, a loader in a beverage cola warehouse, and two coworkers were working outside in the lower parking lot near the trash dumpsters and a fence that ran east to west. The outside conditions were sunny and dry. There were three or four trash dumpsters placed in a straight line, perpendicular to the fence. Employee #1 was walking south on the west side of the trash dumpsters in route to get some cardboard. Coworker #1 was operating a fork lift and had a load of two pallets. One pallet had empty plastic trays and the other had a couple of cases of product. Coworker #1's view was not obstructed, and he was on the east side of the trash dumpsters traveling west. Employee #1 walked out past the last dumpster and was struck in the ankle by one of the pallets. Employee #1 lost his footing and fell into the forklift's mast. Employee #1 sustained lacerations and a fracture to his left eye socket and was hospitalized.
