Investigation Summary
At approximately 1:00 a.m. on January 23, 2007, Employee #1, a contract employee working as a nighttime sanitation worker was delivering trash using a forklift (TCM, Model Number FC625NGT). Employee #1's supervisor did know that he was not certified to operate a forklift. Employee #1 was walking along the left side of the forklift and turned the wheel to the right causing the rear tires to turn into him, toppling him and pinning him under the vehicle. Employee #1 suffered bruises and abrasions and was hospitalized until January 28, 2007.
