Employee Fractures Finger in Cookie Making Machine

At approximately 9:45 p.m. February 16, 2006, Employee #1 a production area employee, was working on a KSC fortune cookie baking machine, Model Number, FCM-8001, located at the north side of the production area. Employee #1 climbed up the ladder near the north side of the machine and adjusted water drops for the molding adjustment. After the adjustment, Employee #1 climbed down the ladder, but remembered that he had to check the depositor to make sure that the dough was coming in from the dough bin, located on the ground level and west of the machine. Employee #1 needed support to prevent him from slipping, so he placed his right hand on an area where there was an unguarded circular revolving part on the same side of the machine, where his ladder was located. Employee #1's hand was caught in the revolving parts. Employee #1 was taken to the hospital and treated for a fracture of the middle finger of his right hand. Employee #1 stated that the guard for the revolving parts on the fortune cookie baking machine was removed so that employees could make constant adjustments. Employee #1 stated that he has worked on the machine without the guarding and has previously spoken to his supervisor about this issue. Employee #1's employer stated that the adjustments have to be made immediately or the machine will stop. The Employer did not develop or utilize a hazardous energy control procedure for employees who are engaged in cleaning, repairing, servicing, setting-up and adjusting the KSC fortune cookie baking machine.
