Investigation Summary
At approximately 6:30 a.m. on August 4, 2003, an employee, a machine operator, was working at the Shinohara Press when the piece he was working on jammed. The piece was 20 feet long and he was simultaneously cutting it into squares and punching holes in it. The piece jammed so he tried to jiggle it. The employee stated that he looked behind him and his hand went in. According to the employee, there was a guard in place but it may have had a wider opening in it than the guard in place. The employee amputated his finger but he was not hospitalized.
