Investigation Summary
At approximately 2:45 p.m. on November 20, 2005, Employee #1, a UTR driver, was driving tractor number 4876, carrying container MSKU 914, which weighed between 27 and 28 tons, containing paper waste. The container was to be delivered to Apple 700 or row 700. The speed limit throughout the terminal in these rows is 15 miles per hour. When the vehicle turned right onto the highline at the intersection of A08 and A09, the vehicle tipped over onto its left side, crushing his skull. According to a coworker, Employee #1 was wearing a seatbelt. It was not possible to determine why the UTR tractor and container tipped over. Employee #1 was taken by paramedics to San Pedro Peninsula Hospital, where he was hospitalized.
