Investigation Summary
At approximately 2:00 p.m. on September 30, 2005, an employee was clearing out his area. As a supervisor, he made the decision to clear out the shipping area. He directed a coworker, a helper, to assist him in clearing space in the shipping area. They were moving pallets of cardboard boxes. Two carpenters from the milling area came to the shipping area to drop off a large platform, approximately 5-ft by 8-ft, made of 2-ft by 4-ft boards. This platform was supposed to be used as a base for a large booth that was under construction. The carpenters stood the platform upright on its side and put some boxes and large sections of wood, approximately 2-ft by 3-ft, against the platform to keep it in place. The employee and the coworker continued to clear the area. Without the knowledge of the employee, the coworker moved the boxes and wood to clear the space and walked away. The employee had his back facing the platform. The platform fell over and struck the employee's right leg and knocked him to the ground. He fell on his arm, shoulders and right leg. The employee's femur was broken in five places and had cuts and scrapes to his arms and shoulders. The employee was hospitalized until October 11, 2005.
