Truck Driver Is Killed by Load of Pipes

At approximately 10:00 a.m. on May 20, 2005, Employee #1 was working as the driver of a flatbed truck, delivering a load of bundles of steel pipe. The bundles were approximately 22 ft long and roughly between 18 in. and 24 in. in diameter. Each bundle of pipes weighed approximately 2,000 pounds. A forklift operator was unloading these bundles when the accident occurred. He had finished unloading the front of the truck, so Employee #1 drove forward to allow the forklift operator to unload the back of the truck. As the forklift operator unloaded the remaining pipes, a bundle rolled off the far side of the truck, striking Employee #1 on the head. Employee #1 sustained a concussion and was killed. The forklift operator had a straight line of sight under the trailer to see the accident. Weather was not a factor.
